Wow. What would it take for them to move back to the city? Closer to medical care for the wife? I’m assuming they are city folk as they call on you so often for help.
It sounds like they think you are their mom living up the road, or something.
They are lucky to have you as a neighbor!

How’s the beagle doing? You are a great neighbor but they are abusing your kindness.
According to the neighbor, the beagle’s doing fine. And I’m the kind of neighbor I’d like to have, I guess.

This. Being a good neighbor/friend/partner/etc. does not mean keeping track every single time.
(I say this with the belief that the OP sounds entirely capable of managing the relationship to her own satisfaction, and will change it if she wants to.)
