Yes this patient would do some pretty sketchy things for a cookie or even a klondike bar ha ha…luckily luvs ear rubs too and that actually gets her to tip her face too thanks for the tip on arm position!!
All I have to offer is commiseration. I had a 12.2 Hackney pony who managed to stab himself in the eye with something (prickly pear spine, yucca, or even a piece of hay–the vet didn’t know how he’d managed it). We thought we had all the prickly pears and yuccas out of the pastures, so it was probably a piece of hay. Anyway, I had my husband do the ointment so Crackers wouldn’t become difficult for me to catch when he was well enough to resume driving. So of course he would come right to my husband, even though he detested the ointment, and I spent the next several months working on getting him easy to catch for me again. This was a pocket pony who loved everybody, but somehow he blamed me for the whole eye episode. He did keep his sight and healed up fine, and started being my sweet boy again after a while.

Glove your right hand, unless L handed. Put a strip of ointment on your index finger, use thumb and middle finger to spread eyelids, place ointment on rolled down eyelid. Use L hand to hold carrots at waist level.

Agree with @EventerAJ I bought a box of 1cc syringes, leur slip tip (that’s important!! Not leur lock) to keep it sterile every time. My mare became such a great patient. Thank god because I was administering her own serum + antibiotics several times a day for at least 8 weeks. I used EventerAJ’s method but made sure to push her upper eyelid slightly back into the socket, then pulled the lower down and place meds in little pocket. It is a labor of love, I tell ya! I treated my mare after every dose, her favorite treat! Good luck.
