Dehiscence...anyone delt with this?

Well, my dog is still with us (see the Dobe could use jingles thread for background if interested) and the seroma (fluid buildup) between the skin and the abdominal wall led to dehiscence. All but three of the staples are removed at this point. I am dealing with a 4" wide by 6" long 1.5-2" deep gaping wound.

The surgeon saw it Tuesday (said it looked “good, gross, but good”) and our regular vet had her Monday. They both said the tissue is granulating and basically it is going to heal from the inside out.

We wrap 3 x a day (flush wound, clean edges, dry, wrap), she’s on 3 different antibiotics (and I’m checking her temp every 12 hours). She gobbles everything I set in front of her like she’s starving. She is feeling REALLY good, seems not to need the pain meds at all anymore (I’m giving her half a pill to make me feel better–2 would be her weight size dose) and I’m having a heck of a time trying to restain her exuberance.

Anyway, I take a picture every day, just so I can track it (if someone is really interested in wounds let me know–my husband thinks I’m weird, but I think it is fascinating) but I’m wondering if anyone has delt with wounds like this before and has any insight into how long they take to heal? I had a wound like this on a mare’s hock and that took months, but it was on the joint (it also wasn’t nearly this big or deep either).

I should add she had her last tiny dose of predinsone last night, so this is her first day steroid-free (steriods are probably what led to the dehiscence in the first place–we have been weaning her off since last week–she crashed after surgery when she hadn’t had them in 6 days–she was on a long acting steriod previously, Vetalog). I realize anyone who responds probably won’t have the steriod issue to, but I’m curious what you have to say!

Weeks to months; with the steroid complication, no idea! Sounds like you’re doing everything with diligence though, and that is the key. I’d say the less time she spends at the surgical clinic the better, even the best of them (mine included) are not fantastic places to have an open wound due to many antibiotic resistant bacterias. We see this type of closure issue sometimes after debridement or in older/obese dogs, though it happens in the young and healthy too. Just one of those cruddy/fluke things. Checkups with the GP would be ideal. I’d like to see the pictures eventually, I can’t be the only curious soul! I think your continued maintenance with the wound dressing and keeping a close eye on temp will be your best tools.

I hadn’t thought about the bacteria at one clinic vs. the other. Pooh. I really like visiting with the surgeon (plus she said if I ever need a two hour break from the constant babysitting I can just drop her off). She and I got on the ground and looked up at “the hole” and she pointed all kinds of stuff out to me, which I really appreciated. I intially thought I was looking at a bit of intestine or something when it first opened up and was rather horrified (her abdominal wall is looking good, holding). Surgeon really loves the dog–told me she would be happy to take her home :wink:

I realized this week after paying pretty extensive bills to both, that the ER clinic is actually much more reasonable than my regular vet. That was a suprise!! But he will remain our GP–we’ve been going to him for years and they are great people too.

I should mention that obedience training “stand” and “stay” have come in beyond handy with an 70 lb dog who needs a belly flush 3x a day. . .

I hope it is weeks!!! I am going through six rolls of vetwrap a day. 3-4 rolls of elastikon a week, a box of surgical bandages, 2.5 rolls of cotton wrapping, you get the idea…I’m buying by the case. I’m hoping at some point (when it is much, much smaller) and it stops draining and she can wear tank tops or something to cover the wound.

I would estimate 2 to 4 weeks. This is based off our experience with healing the dehisced radical masectomy. I used diapers with sugardine with a brown gauze and vetwrap on her.

My dobe is also an eater though not to the extent your’s is. Mine got hold of a bottle cap and ripped up his intestines. Lost 6 inches of it and we were lucky. If we hadn’t agreed to exploratory surgery we wouldn’t have found it as if did not show up on x-ray since the plastic was small and blended in with the rest of this body. His symptoms were diarrhea (sometimes leaking poo water out his bum while just standing or lying around, throwing up, and very depressed listless. Went in the first time and thought maybe he had guardia or something again and that he was really dehyrated and gave him IV fluids, then said to come back if he didn’t seem better. He obviously didn’t. Next time we took x-rays, didn’t see anything so went on to surgery. He spent a couple nights at the vet (the first at the regular vets as he was too critical to move, but they cant have people in the clinic during the night so that was a bit scary) so then we took him to the emergency clinic for the next night where people would keep an eye on him for the whole night. Luckily he healed up well and as easily as he could.

Its a “routine complication” of GI surgeries. Most need to be re-cut, but sounds like your dobe is tolerating it by bandaging/medical management etc. Thats a great sign.

Keep an eye out for anorexia, fever, lethargy etc. as these are signs that internal dehiscence is happening. Usually if you make it to 6 days post op you are out of the woods.

We are 13 days post op today! She is roar’n to go. It is increasingly difficult to restrain her exuberence.

The wound started contracting in size about 4 days ago. Yay! It looks very clean and the inside has a couple layers of scar tissue. Drainage slows a bit every day as well.

Hi, TrotTrot…

My now-deceased coonhound had a massive tumor on the front of her chest - right where her right front leg “connects.” The vet had to go in and remove (thankfully not cancer), and she had to really go in deep. I had had no experience with this kind of wound/surgery and so I let her have WAY too much activity right after surgery. This led to sutures and staples having to be removed, and the wound looked much like your dog’s. I can’t tell you how long it actually took to heal - but it too healed from the inside out - it took months at least. It sounds to me like you are definitely on the right track, and from how clean her belly looks, you are doing a wonderful job keeping her clean! Good luck!
