on your third refusal (or 12 faults) your disqualified but how many refusals can you have at one jump? If your horse refuses the same jump twice are you disqualified??

Yes. You can only have two refusals before you are excused. Doesn’t matter if it is the same jump or different jumps. A refusal can also be that you circled (crossed your path), or any stoppage of forward motion regardless if where on course (for example your horse doesn’t want to go past something scary or through a gap and stops)
Nope. On your second refusal you are done.
Used to be 3, changed to 2.

Except for jumper classes restricted to horses five years old and under, where it’s still three refusals. See USEF rule JP 144.
and I would suppose horse leaves arena with 2 refusals on his brain? no ability, except maybe at schooling shows, to jump jump #1 before leaving? takes too much time I guess. I understand the reasoning, just not in agreement.

See USEF rule: JP 135.8. Courtesy Fence. An eliminated competitor has the right to make one attempt to jump an additional single obstacle, which the judge(s) may designate, but may not continue thereafter. (Exception: The right to attempt an additional obstacle does not apply to Fault and Out classes or in the case of the fall of the horse or rider.)
