How to create an entry for a closed event

I emailed the secretary to see if I could get in at Pine Top which closed March 1st. Thery said to email them an entry and they would try to fit me in. How does one find a blank entry form? I was too embarrassed to ask the secretary.

Evententries will also make a printed entry form for you to email the secretary I believe (I’ve never done it, but I see the option)
