Does anyone have any recommendations for how to ice/use cold therapy on hard-to-wrap areas such as the stifle or elbow?
Ice Horse sells a stifle wrap! I’ve been using it for a couple months now. Works well and beats cold hosing by a long shot.
I also recently purchased the stifle wrap and it works well! But there was absolutely NO WAY to get it on as shown in the videos unless I reversed left and right. I had several people try, and we were watching the video in the paddock trying to get it on right. I finally concluded the labeling was reversed 
We use frozen peas for tricky spots, bandaged in place best we can, or holding them there if necessary.
Frozen peas will form nicely around and into all kinds of places.
