My horse suffers from onchocerciasis.
At the beginning of the year he received 3x Dectomax every 2 weeks. Since no veterinarian knew what it is, so precautionary Dectomax was injected. 3 weeks ago, then came a new vet and knew immediately what it is, he knows Filairen apparently. He advised me to give the horse a dose of Equimax, but I read on Facebook that 2 would be better and have done this, the vet knows about it :-). Now a week ago he gave me Equest Pramox (Moxidectin), again double the dose. For 3 days shows a very large Wikrung, that is, the horse has rash, weeping, itching …
The vet said that if he responds so well I have to give one or two worm treatments in one week. Drafting Equest (Moxidectin) or Equimax …
I found on the internet that you can no longer give ivermectin after moxidectin and because moxi only works longer every 3-4 weeks.
Mixing ivermectin and moxidectin? Weiss did not really think so …
What would you advise me?

Do you mean you shouldn’t give ivermectin so soon after moxidectin? I would agree with that.
Do you mean because moxidectin works longer than ivermectin, then you should only give it every 3-4 weeks instead of every 2 weeks? If so, it is true that moxidectin works longer than ivermectin, but the context is a bit different. Ivermectin works longer than 2 weeks as well, but in the context of strongyles, not neck threadworms. The double dose of Equimax (or plain ivermectin sometimes) every 2 weeks is based on the juvenile NTWs, and trying to get ahead of their development.
Going forward I would leave moxidectin out of this. Me, I would wait at least a month to give him anything else. And unless he was just miserable, I’d probably wait 8 weeks, get more of that moxidectin out of his system. I don’t know that there’s any formal protocol for that, but that’s just what I would do.
There are some old threads about this issue on the COTH forum. The worms get in the nuchal area of the neck and it takes a week or more of doubled up Ivermectin or Quest to get rid of it. I would search for that information. Alot of people don’t know about this problem. Good luck!
Texas A&M recently had me give a dose of ivermectin 7 days in a row followed by 1 dose 2 weeks later to treat.

OP, IMO the advice you received was not good.
I put two tubes of pure Ivermectin down my 13.3H Arab at once. In a few days he had a horrible skin breakout from the microfiliae dying off. I kept the sores cleaned and applied diaper rash paste with 40% zinc oxide to help with healing and itching.
I also orally fed him injectable cow Vitamin A for the rest of the summer. The dosage is important but I can’t remember it by now.
His winter coat came in a richer deeper bay and was very soft. He never had a repeat of thread worms until his passing at age 29, which was 3-4 years later.
i agree that I would not shove any more worm meds down your horse for a long time and ditto moxidectin is NOT what is supposed to be used:)
Vitamin A? why? have not heard of this.
How was this condition originally diagnosed in your horse?
I have heard that NTW is a regional issue, but don’t recall which regions are problematic. Enlighten me?
To hopefully boost his immune system. I forget how many Cc I gave him and how often. I know it was NOT every day and was spread out over the hot months when midge flies are the most active.
I only gave it to him that one time when I dosed him for NTW’s, which he never had issues for the remainder of his years, (RIP my precious Streeter).
