Some thing I should have included in the lease for my 18yo PSG gelding:
Yearly vet check with flexions etc to assess possible need for injections etc
And be sure to include payment of insurance if he’s insured, and use of all supplements he is currently on.

Some things I considered, have done two of these: One horse was older and one had a colic history.
Both were written as month-to-month, but I required 1 full calendar month of notice to end lease (ie you call me on 8/18/23, you are responsible thru 9/30/23. This was because I board and would need to be sure stall available.
Second, in both cases I took $$ responsibility for major health issues and any decisions related to them. The other party was responsible for ordinary vet and farrier stuff - ie shots, worming coggins, minor cuts/scratches, etc.
Liability clause to put burden of any damage or injury caused by said horse was not my problem.
The old guy had a very happy 18 months doing lower level dressage and some natural horseman stuff before he showed some neurological signs. They called me, I picked him up and brought him back to my boarding place a couple days later. Cervical arthritis.
The colic problem child went a year or more colic free but then the trainer relocated his business and the person leasing chose not to follow.
Google a horse lease agreements and you can piece one together pretty easily.
one other thing to consider: make sure that the person leasing the horse will authorize you to have access to the vet records of whatever vet is used by them. Just in case…
