Not sure if they go that far but Equine Veterinary Hospital of Northern Indiana (I have sport horse experience with Dr. Randolph and Coyle and recommend both). Second choice would be Conley & Koonz but they might be closer.

Conley and Koontz are pretty close. Another is Dr Hammond out of Weldy and Associates in Wakarusa. Both are reputable.
