PSSM Type 2 and EPM

As some may know, my grey mare was diagnosed with EPM in November after she fell with me while riding. The vet came out two days later and did a full neurological exam, flexions in her hind end (which he said were fine), and pulled blood to run a titer test. Her titer came back at 35 I believe and her CRP was elevated as well so we began treatment with Levamisole and Dequocinate (sp?). She seems to be doing better, although I have not had her blood pulled again yet but I do plan on having that done soon when the vet comes for Coggins and spring vaccinations. I have suspected that this mare may also have PSSM type 2 as she seems to show several symptoms: moodiness about being saddled/ girthed (has been treated for ulcers, seen by Chiro, etc), constantly rubs her haunches on things despite having a clean fecal and doing a 1.5 dose of Anthelicide, stiffness on one side (left), sensitive to grooming (blown off as thin skinned tb), staying camped out for a long period after urinating, holding tail stiffly, drags toes while being ridden (EPM?), lack of impulsion, difficulty picking up canter or maintaining a lead (attributed to muscle loss from EPM), stall walking, muscle tremors (left foreleg), difficult to put topline on/ keep despite being in good work and being worked well (not in a lot of work at the moment due to my schedule), pinning ears while cantering, does not like to have hind legs lifted a decent height for shoeing/ cleaning hooves, sweats a LOT (like sweating while standing when its 80 degrees), and difficulty bending/ collecting. I am sending in a blood sample to Equiseq for testing. She is currently on a PSSM (almost) diet she gets:
2 scoops (1 serving) California Trace Plus
2 scoops (1 serving/ 8,000 IU) Natural Vit E pellets from Santa Cruz Animal health
2 scoops (1 serving) Quiessence
4 cups Alfalfa pellets
All of the above is split equally between two meals - She does still get her EPM medicine once a day

I purchased Canola oil last night and will be adding it to her diet. She has been on it before at a cup a day and was doing well, but I didn’t think she needed it to maintain weight (she is at a good weight, but has no topline). She is also on pasture board and is out 24/7 but has been blanketed all winter (she is not clipped). She does seem to need a longer warm up on the colder days, but I attributed that to being almost 13 and having a few previous injuries.

So my questions are: What else can I do (besides more regular work) or give her to help her (change her diet)?
and most importantly
Could the suspected EPM really have been PSSM?
I know in our area (Missouri) almost every horse will have something show up on their titer, and 35 didn’t seem that high to me, but EPM seems to be stupid common around here (like 50% HAVE to be misdiagnosed - there were 5 horses in a 14 stall barn that were diagnosed with it).

PSSM2 horses need higher protein, not fat.

bugsynskeeter - I had read that they need moderate fat with high protein. I am looking into protein supplements now, but I am also trying to avoid soy with this horse.

Alfalfa pellets are a good source of protein, so you are in the good there. I would still try the fat and see where that gets you.

I don’t know much about type 2, my own horse is type 1, but if I remember correctly Quiessence contains Chromium? Chromium is counter productive to controlling PSSM symptoms. I would suggest getting her on a high dose of magnesium. My own horse gets 10g a day.

I recently added MSM to my mare’s diet. It can help with muscle pain as well as joints. I can’t say yet if it is helping as we have a lot of other things going on right now as we try to figure out a hip issue, but I know a lot of other PSSM owners say they felt a difference. It’s cheap enough that I am willing to give it a try.

Perfect. Most see PSSM and think just low carb and high fat. But definitely need the increased protein with type 2. I’ve seen several people just use a whey protein for added protein (instead of a typical horse supplement).

Draftmare - I’ve had her off and on MSM a couple times (and a few different brands) and it doesn’t ever seem to make a difference, although when she was on fish oil she was less inflamed over all but I’m not sure if that would make any difference with the PSSM. The alfalfa pellets are good for protein but I don’t feed enough of them to make a difference - she gets about 1.3 lbs a day. I would like to put her on all alfalfa or grass alf mix, but she is in pasture board and the barn supplies the round bales (and I don’t want to spend a bunch on alf for someone else’s horse to eat it out in the field). The alf pellets are mostly to make her feel like she’s getting a “meal” and to mix with her supplements.

bugsynskeeter - Like human whey protein powder? I’d love to find something pelleted as she isn’t super fond of powder (and I feel like less is wasted) but the powder might work, especially with the oil so it sticks.

Although powdered, TriAmino or Nutramino are good supplements. The volume is not all that high. I think they are kind of stinky, but my horse eats it right up with soaked food. You could get HorseTech to make you a flax based supplement with magnesium and Nutramino. Might be more palatable that way and combine 3 things you need (some fat, aminos, and mag).

IPEsq - We’ve done the custom supplement before but it was more expensive and was such a high volume of powder. It’s also easier for me to do them separately as I have my other mare on the same products but at different levels. We’ve done tri-amino before, but I don’t remember being overly impressed with it. That being said I can always go back to it and if nothing changes at least it isn’t expensive.

Anyone have thoughts on whether the EPM “episode” I guess you could say might have been PSSM???

Also, would it benefit her to start her on Acetyl-L-Carnitine? I have found some information but not a ton. I know its supposed to be fed with a lower fat diet (not sure why) and that it doesn’t help all horses, but the ones it does help show results very quickly.

A better place to ask these questions may be the the PSSM group on Facebook. Lots of knowledgeable people there, and they tons of articles pinned for you to read.

I am already a part of that group. The majority of the information there seems to be regarding type 1 and I did not find any new helpful information in their pinned articles. I am also wanting an answer to my EPM question (which I have already asked there as well).

Bump - Still hoping someone might having insight on the EPM possibly being PSSM
