Does anyone have a source for such a pattern? I searched a bit on line and couldn’t find one.
Suitability Patterns makes a pattern for a shadbelly that includes vest points:

I haven’t made this pattern, but I have made the Suitability 7900 pattern and its instructions were straightforward to me (I have intermediate sewing skills.)
For a pocket square, you don’t need a pattern. Just cut a 12-14 inch square of the fabric of your choice. If you use silk, you probably will want to do a hand rolled hem, as shown here:

If you use cotton or handkerchief linen, you can fold the raw edge over twice, about 1/8 inch margins, iron the folds, and then machine sew it to hem.
Thank You!
