Squamous and Glandular Ulcer Treatment

I have a gelding that was diagnosed with both squamous and glandular ulcers. The vet recommended both the omeprazole and misoprostol for the treatment plan. He is already on a management plan for ulcer care (pasture board with a run-in shed, 24/7 good quality hay, no grain) as well as I added 4 Nexium a day. My question is what others have experience with the misoprostol or is there other suggestions for help with this especially the glandular ulcers. His stomach ph was 2.2 tested during the scope

where was that pH? It should be that low down near the pyloric valve, but closer to 4 as you get to the top

Are you saying you’re adding 4 Nexium on top of omeprazole? I wouldn’t. They do the same thing, so you’re just “overdosing” the proton pump inhibitor aspect.

Did your vet mention sucralfate as well, to coat the existing ulcers to make it easier for them to heal?

A combination of Misoprostol and Omeprazole was key for my guy who also had squamous and glandular ulcers. His pH was 1.6 at first scope. 30 days of just miso yielded almost no improvement.

Followed up with another 30 days of higher dose miso (2600mcg AM + PM) and added 1 tube of GG in the morning. He improved from Grade 4 to Grade 1 at rescope. We continued this cocktail for another 30 days to fully resolve.

I am not sure where the ph was I will ask the vet. No I am only doing the Nexium not both and no she did not recommend sucralafate she said misoprostol instead.

Not trying to penny pinch but is there cheaper way to treat the glandular ulcers besides the misoprostol

I don’t know how it compares to what you’ve been quoted already, but I got all our misoprostol through Rood and Riddle’s compounding pharmacy. They offered it in a variety of dosages and forms, and the cost worked out to about half what the same quantity of tablets would have been.

When my horse was diagnosed with squamous and glandular ulcers we treated with omeprazole and sucralfate. Rescope after 30 days the glandular ulcers were pretty much healed, squamous much improved, so we did another 30 days.
